Dutch Melanoma Treatment Registry (DMTR)
Ieder jaar krijgen ongeveer 8.000 Nederlanders een melanoom. Melanoom is een type huidkanker dat ontstaat uit pigmentproducerende cellen. Melanomen ontwikkelen zich meestal in de huid, maar ze kunnen ook voorkomen in andere delen van het lichaam, zoals de ogen en slijmvliezen.
Kwaliteitsregistratie DICA Melanoom
Via onze kwaliteitsregistratie DICA Melanoom (DMTR) verzamelen we gegevens van patiënten met een vergevorderd en uitgezaaid (naar de lymfeklieren of andere organen) melanoom. De zeer specialistische zorg voor deze patiënten is geconcentreerd in 14 melanoomcentra binnen Nederland. Al deze melanoomcentra doen mee aan de DMTR.
De afgelopen jaren zijn er nieuwe vormen van geneesmiddelenbehandeling beschikbaar gekomen, zoals immunotherapie of doelgerichte therapie. De melanoomregistratie geeft inzicht in de uitkomsten van deze nieuwe behandeling, ook voor kleinere of ondervertegenwoordigde patiëntengroepen. Hiermee stimuleren we verbetering in de behandeling en dragen we bij aan het ontwikkelen van expertise over nieuwe geneesmiddelen. Bovendien dient de registratie als platform voor het verrichten van doelmatigheidsonderzoek naar de behandeling van uitgezaaid en vergevorderd melanoom.
De DMTR is in 2013 in samenspraak met de Nederlandse Vereniging voor Medische Oncologie (NVMO) opgezet en maakt deel uit van de Dutch Melanoma & Skin Cancer Group (DMSCG).

Resultaten
- De mediane overleving van behandelde melanoompatiënten met systeemtherapie is gestegen van 11 naar 32 maanden van 2013 tot 2019.
- Eerder stoppen van anti-PD1-behandeling lijkt niet ten koste te gaan van de effectiviteit.
- Ook patiënten die niet voldoen aan de inclusiecriteria van de registratie ondervinden meerwaarde van de nieuwe behandelingen.
- Patiënten met een acraal of mucosaal melanoom hebben minder baat bij de nieuwe melanoombehandelingen.
- Behandeling van de bijwerkingen van immuuntherapie is mogelijk van invloed op de effectiviteit van de behandeling met checkpointremmers.
- Tussen de verschillende melanoomcentra is er geen significant verschil als het gaat om de casemix gecorrigeerde overleving van melanoompatiënten.
Patiëntgroep
In deze registratie verzamelen we gegevens van alle patiënten met:
- een irresectabel stadium III of stadium IV-melanoom (inclusief uveaal melanoom). Het betreft hier patiënten waarbij het stadium III of stadium IV van de ziekte wordt vastgesteld als primaire diagnose (synchroon), maar ook waarbij het stadium van ziekte zich in een later stadium dan de primaire diagnose manifesteert (metachroon);
- een stadium III of IV-melanoom die behandeld wordt voor (neo)-adjuvante behandeling en gezien zijn in een melanoomcentrum.
Niet cytologisch of histologisch bewezen melanoom valt buiten deze registratie.
Patiëntperspectief
Wij nemen Patiënt Reported Outcome Measures (PROMs) op in onze registratie. PROMs zijn gevalideerde vragenlijsten waarmee we gestructureerd informatie kunnen vastleggen over de kwaliteit van leven van patiënten. Voorbeelden van generieke uitkomsten zijn fysiek functioneren, vermoeidheid en mentale gezondheid. Ziekte-specifieke uitkomsten bij melanoom zijn klachten en bijkomende zorgen na een melanoomoperatie. Daarnaast is de patiëntvertegenwoordiger vanuit de Stichting Melanoom is onderdeel van de wetenschappelijk commissie en praat inhoudelijk mee over de ontwikkeling van de kwaliteitsregistratie.

Leren en verbeteren
Via onze dashboards krijgen deelnemers aan DICA Melanoom wekelijks een geactualiseerde terugkoppeling van hun eigen resultaten, afgezet tegen het landelijk gemiddelde (benchmarkinformatie). Terugkoppeling vindt wekelijks plaats via Codman Indicatoren, Codman Beschrijvingen en Codman Exploratief
